How to change the order of questions on a form:
1. Select the Forms icon.
2. On the Active Forms list, locate the form you wish to edit, and click Edit on the form's info bar.
3. To the right of each question you will see an Up Arrow, Down Arrow, Plus Sign, and Copy icon.
Up Arrow - move that question up one line
Down Arrow - move that question down one line
Plus Sign - add a new question above the current question
Copy icon - copy the existing question and answer set and paste at the bottom of the question list
4. Choose the appropriate moves. Changes are saved automatically.
