This page details how to edit the content of the form as displayed to clients prior to the client completing the form.
Note: Editing and deleting saved Forms is disabled to comply with HIPAA regulations - if you need to edit/delete a form, please contact your personal account manager.
How to edit a question on a form:
1. Select the Forms icon.
2. On the Active Forms list, locate the form you wish to edit, and click Edit on the form's info bar.
3. To the left of each question you will see pencil icon, click the pencil icon to edit.
4. Make the necessary changes and click Save.
