For counselors submitting paper claims, you will complete the 1500 form online, save as complete, and then print directly on a red 1500 form (not a blank piece of paper) by clicking the Print to CMS 1500 button at the bottom of the claim.
Trouble-shooting CMS 1500 printing alignment
If the info fields are not aligned when you are print to the red 1500 form, follow these steps:
1. Check to see that the "Scaling" and "Fit-to-page/Shrink to fit" printing options are disabled on the print preview page. These features will distort the fields.
2. Move the fields to the right or down by increasing the top and/or left margin. Go to Settings > My Settings > Display > Insurance Claim 1500.
Note: As printer settings vary between manufacturers, some trial and error may be required to determine the correct number for adjusting the printing fields.
